LEVELS OF CARE

ROUTINE LEVEL OF CARE

Routine hospice care is provided in the patient’s choice of residence by a designated caregiver and/or family members with the support of the hospice team. Most hospice patients are managed under this level of care.

Choice of residence:

  • at home
  • nursing or group home
  • assisted or independent living community
  • retirement or long-term care facility

In a facility setting, hospice and facility staff work together to develop a care plan that will meet the changing needs of both patient and family.

INPATIENT CARE at Gilchrist Center towson, Gilchrist center howard county or Gilchrist center Baltimore - joseph richey house

Inpatient care may be provided to help manage complex pain and other symptoms.

This patient will require:

  • Skilled observation and monitoring which may lead to changes in the plan of care
  • A complex treatment schedule
  • Pain management and symptom control
  • Frequent parenteral injections
  • Intensive education

CONTINUOUS CARE

Continuous care is provided when patient’s needs are similar to those requiring inpatient care, but the patient/family chooses to remain in the home setting. Skilled care is provided for a minimum of eight hours per day in the home setting.

RESPITE CARE

Respite care is short-term care provided in a variety of selected area nursing facilities when a caregiver needs a rest from the demands of home care or needs to leave the home for an extended period of time. Under most payers, respite care can be provided for up to five days at a time.
