An interdisciplinary, specially trained team of professionals and volunteers, working in close cooperation with the patient’s personal physician, plan and deliver all services tailored to each patient’s and family’s special care needs.

You and Your Loved Ones
You are the most important member and leader of the team. Our focus of care is in response to your choices for comfort.
Your Physician
Your physician of choice is an important member of the hospice team. Your personal physician can continue to care for you and work in collaboration with the hospice team or you may choose to designate the hospice medical director as your primary physician.
Your Nurse
Your nurse provides skilled care and support by understanding your illness and listening to your wants and needs. Your nurse will also provide education and offer options to manage your illness and support your caregivers.
Your Home Health Aide
Your home health aide will assist you make every day tasks easier – bathing, grooming, dressing – while at the same time, helping you maintain your dignity and independence.
Your Social Worker
Your social worker provides support and counseling for you and those close to you and information to assist you with other practical concerns, such as alternative living arrangements, financial issues and referral to other agencies.
Your Volunteers
Your volunteers share companionship with you and your loved ones. They can provide respite for caregivers by sitting with you while your family takes a break or listening and sharing things you enjoy most. Your volunteer’s special talents are matched to your needs.
Your Chaplain
Your Chaplain provides spiritual support and counseling for you and your loved ones based upon your spiritual beliefs, practices and needs – listening and offering prayers, reflecting about things you find most meaningful in your life, and support in healing relationships. Your chaplain also works with your own clergy and congregation if you desire.
Your Bereavement Counselor
Your bereavement counselor is available to assist your loved ones as they grieve. Bereavement services include individual counseling, support groups, periodic phone calls and written resources.
Other professionals, including physical and occupational therapists and nutritionists, are available as needed.
